Some families may have both Non-Medicare and Medicare eligible participants. The rates below reflect the costs of enrolling your family in a combination of two plans (1 Medicare Plan and 1 Non-Medicare Plan). Please refer to the information under the Non-Medicare and Medicare links above to research the individual plans listed in this rate sheet.
